Home Care Services 
Community Health Centres
Community Health services vary from centre to centre and include family health services, mental health services, child health services, HACC, community health nursing services and confidential counselling. Referrals to self-help groups and health welfare services, drug education and drug dependence programs may be available. Physiotherapy, occupational therapy and Aboriginal and Torres Strait Islander services may also be available. Some Community Health Centres offer access to Home and Community Care (HACC) programs. HACC programs are a joint state and federal government initiative which provides frail aged people and people with a disability access to practical services which assist them to continue living in their own home. Support services are also available for carers. Services which may be available to eligible clients include domestic assistance, social support, home nursing and personal care, respite care (in home and centre - based), home maintenance or minor modifications, allied health services (such as podiatry, dietetics, physiotherapy and speech pathology) and referral to other support services where needed (such as Meals on Wheels). Fees apply to certain HACC services, but special arrangements may be made for people who cannot afford these costs.

Caring Connections offer persons dealing with a life-threatening illness volunteer services such as companionship, respite services, transport to medical appointments, shopping, taking client and carer on outings, practical support such as help with light washing, washing up, snack preparation and errands, socialisation and recreational activities, and massage of hands and feet of client. Volunteers continue to visit and support client and carer when client is admitted to one of the partner's facilities eg. Mt Olivet hospice. Caring Connections also provide access to physical resources and information such as books and videos, carer education sessions, carer and client support groups, relaxation classes, art and music therapy groups, group outings, video evenings, access to counsellors and complementary therapy for a reduced fee.

Cleveland Baptist
Cleveland Baptist Church provides Pastoral Care and support, professional counselling for families, couples, children, youth groups and play groups.

Community Health - Inala
The services provided by the Inala Community Health Centre, QEII Hospital Health Service District, Queensland Health include dental health, family health services, mental health services, child health services, HACC, community health nursing services, alcohol and drug services, diabetes education and other health education, confidential counselling, General Practice, Indigenous Health services, developmental assessment and therapy services, physiotherapy, occupational therapy and podiatry. Referrals to self help groups and health welfare services are provided where appropriate. The Inala Community Health Centre offers access to Home and Community Care (HACC) programs, which provide frail aged people, people with a disability and the carers of such people access to practical support services that assist them to continue living in their own home. Developmental Assessment and Therapy Services are provided by a multi-disciplinary team providing assessment and intervention programs for children with developmental and learning problems, as well as support to their families. The service also provides educational programs within the community on issues related to child development. Free general Dental treatment is available to eligible patients (Queensland residents with a health care card, pension card or seniors card). The Centre provides a General Practice. It is unique in that the Practice is jointly managed by both the QEII District and the Centre for General Practice, University of Queensland. Primary services provided by the Practice include family medicine, emergency services, paediatrics/immunisation, antenatal care, pathology collection services, counselling, alcohol and drug (detox), home visits, clinical pharmacy consultations and counselling and education by nurses for diabetes, blood pressure, asthma and smoking The Inala Community Health Centre also has access to interpreters and offers the services of general practitioners and a Spanish interpreter on Wednesdays 9.00 am to 12.00 noon, and the services of general practitioners and a Vietnamese interpreter on Tuesdays 9.00 am to 12.40 pm.

Community Options - Sth Bris
The South West Brisbane Community Options provides a range of flexible support programs for the frail elderly, younger disabled and carers. This includes: * Domestic Assistance * Social Support * In Home Respite * Personal Care * Meal Preparation * Banking * Shopping * Case Management and * Advocacy. Other services available include a Homeless Program, Community Aged Care Packages, Community Options Package and Department of Veterans Home Care Packages. Community Options also provides the following services at other off site locations: * Centre based respite for residents of Centenary suburbs - (07) 3376 4841 * Support to the homeless or those living in marginalised housing. - (07) 3844 8808

Karuna Hospice
Karuna Hospice Service is a not-for-profit public benevolent institution and registered charity, formed in Brisbane by health professionals and community members. The staff and volunteers offers comprehensive support for the physical, emotional, social and spiritual needs of the individual. By taking hospice care into the home Karuna offers choice and promotes quality of life. Karunas philosophy is based on Buddhist principles of love and compassion. The goal of Karuna is to offer people with a terminal illness, and these people's loved ones, to enjoy the highest quality of remaining life. Karuna provides home palliative care, loan of nursing equipment, in home respite care, counselling and ongoing bereavement and spiritual support.

Mount Olivet Community Services is a Catholic, non-profit health and aged care organisation under the care of the Sisters of Charity. Its aim is to provide care to the whole person, with acceptance, compassion, and the highest standards of appropriate medical and nursing care, irrespective of the patients religious, social or cultural background. southern cross
Southern Cross Care Community Services was established with the prime objective to provide a comprehensive and integrated range of home care and support services to assist people to live independently in the community. Southern Cross Care Community Services works independently and in collaboration with many of the community groups to structure services that meet individual and family needs. A range of services provided by Southern Cross Care Community Services include: * Nursing Services: General nursing all levels, palliative care, pre and post hospitalisation, wound care, continence management, case management and co-ordination of care. * Personal Care Services: Providing personal care for all activities of daily living, providing personal care for very complex and high care needs, providing personal care for all levels of disability. * Home Help: Domestic services, meal preparation, shopping and transport. * Respite Services: Services to support the family in caring for loved ones at home. * Equipment - sales and hire, relief staff and support staff for other community agencies. Networking and resources services are also offered by Southern Cross Care Community Services. Southern Cross Care Community Services provides the highest quality of service at the lowest possible cost.

Zoe Reed Little
Zoes Place Family Support Service assists families who have a child with a life limiting illness, presently at home with trained volunteers. A centre will be constructed at Mount Ommaney for respite and hospice care.
